A Virtual, Contact-Free Check-In Process
We are excited to announce that we now offer a new online registration and check-in application that allows our patients to complete their intake process on their phones, tablets or computers prior to their appointment.
What is Phreesia?
Phreesia is an application that manages the patient intake process, including mobile registration and payments. Mobile check-in allows you to conveniently complete pre-visit registration from your own device and in the safety of your home, car or another personal space.
Why did Clay Eye Physicians & Surgeons partner with Phreesia?
Now more than ever, Clay Eye Physicians & Surgeons recognizes the importance of safety. Phreesia is a platform that promotes safety by allowing you to complete information prior to your visit, limiting touchpoints in our clinics as well as time spent in our waiting rooms. In addition, Phreesia ensures that your information will always be up to date. The information you enter is private and secure.
How it Works
A few days prior to your scheduled appointment, you will receive an email or text message from Phreesia, our new check-in platform. Simply click on the link that will be provided. You won’t need an app, user name, or password and you will be guided through the check-in process. It’s very user friendly.
Phreesia allows you to register for your appointment from the safety of your own home, reducing your wait time at our office. It also allows for you to complete this process when it’s most convenient for you.

Is the Phreesia system secure?
Yes. Phreesia is held to the same standards as Clay Eye Physicians & Surgeons, relating to the Health Insurance Portability and Accountability Act (HIPAA), which protects your health information. For additional information about Phreesia’s security, visit www.phreesia.com/patient-privacy.
Do we need to use the new system for every appointment?
Yes. Once you have completed the initial registration process, Phreesia will save your answers. The next time you register for an appointment, the process will be much quicker, as less information will need to be entered.
Do I need to download an app to participate with mobile check-in?
No. You won’t need an app, user name or password . You will be sent an email or text message, and you can simply click the link provided and you will be guided through the check in process.
Can I complete the forms in a language other than English?
Yes. Mobile registration is available in 20 different languages.
What happens if I can’t finish my registration before the appointment?
If you’ve started the process and weren’t able to finish, don’t worry, Phreesia saves the information that you have already entered. Please see one of our patient service representatives, and they will send you a new link that will allow you to complete the remainder of the registration.
Do I still need to check-in when I arrive?
Yes. When you arrive for your scheduled appointment, please check in at the front desk. You may choose to wait in our waiting room or your vehicle, whichever you are most comfortable with. We will notify you when our team is ready to see you.
