Required Paperwork for each visit
On your visit to our office, we recommend you arrive 15 minutes prior to your scheduled appointment time. If you need help locating us, please click here. When you arrive, you will be greeted by one of our reception desk employees who will ask you to provide some required information.

Filling out these forms generally takes 10-15 minutes. For your convenience, we have made the Patient Information Form available for you to fill out before hand. We recommend you view the form in Adobe Reader v6.0 or better. You can type your information directly onto this form. Please print it out and bring it with you to your appointment. Please remember that an updated form and signature may be required by your insurance company on every visit to our Practice.
Dilation Information
It may be necessary to dilate your eyes during the course of your eye examination or treatment. Dilation results in sensitivity to light and an inability to see well at close range or distance for a few hours. We provide free disposable sunglasses or dark sunglass inserts. Patients should wear sunglasses and be cautious walking and going up or down stairs. We recommend avoiding driving or operating dangerous machinery immediately afterwards. We recommend that someone accompany you to drive you home or that you wait until your eyes return to normal so that you can drive safely.
Refraction Information
Depending on the nature of your visit, your doctor may determine that it is necessary to perform a refraction. This is a valuable test that determines whether your blurred vision is caused by a medical condition or is a simple need for new glasses or a contact lens prescription. Most insurance companies don't pay for this portion of your eye exam. Contact your insurance carrier or our office for more details.
LASIK eye surgery can be an affordable alternative to glasses and contacts. Our Florida LASIK eye surgery center goes way beyond what other centers do to help our patients overcome this affordability issue. If cost is something that has been holding you back from vision correction you should really contact us as soon as possible to set up a consultation.
Cataract patients now have an option to see at both near and far distances after
cataract surgery! Individuals suffering from cataracts previously had only a mono-focal lens implant option
after surgery. NEW multi-focal IOL technology can now decrease dependence on glasses after surgery.
Cataract surgery utilizes an intraocular lens (IOL) to replace the aged, cloudy
natural lens of your eye. An IOL is an artificial lens that is implanted inside your eye during cataract surgery.
The standard procedure uses a "mono-focal" IOL. This type of lens is very effective at restoring functional
distance vision.

In August '08 we opened the Macular Degeneration and Diabetes Eye Institute of Northeast Florida which operates adjacent with our existing location next to Orange Park Medical Center.
